We don't bother earthing out LV circuits but anything MV and above is not deamed Isolated until earths are applied to the circuit.

i second what Dave said...we'll perfrom voltage checks on the LV side and then ground out the MV systems.

Has much to do with context. Some facilities are more comfortable with a visible open; i.e: withdraw the breaker; but on systems with overhead distribution lines (exposure to accidental energization or induced voltage) grounding is the norm, always.
